(Video) Endoscopic Cystolithotomy with Mesh Resection
Abstract
Introduction: Complications of retropubic sling insertion are mesh extrusion (2.7-4.4%), voiding dysfunction (3%), urinary tract infection (10-17.2%), neurological symptoms (5.4- 8%). Approximately 42% of the patients suffer one of the mentioned complications and 12% a serious adverse event. At one year and 9 years, complications will be between 2.3% and 6.1%. The most com- mon are mesh exposure and pain. One of the important aspects in the management of complications is to be able to suspect and diagnose them in time. The management of the extrusion of the mesh is an important aspect in the complications since it implies an invasive treat- ment in its resolution. Objective: The objective of presenting the following video is to present a rare case of mesh resection attached to a stone through the vagina or abdomen, complementing the fragmentation of the stone with lithoclast, to contribute to the experience of the area and demonstrate the technique applied to complement the different methods used in this case. Methods: Method: The patient’s consent was obtained to record the surgery on video. The images were obtained directly by recording them from the pavilion and they were complemented with the videos obtained from the cystoscope to edit them in Adobe Premiere. Results: no results Conclusions: Conclusion: The adequate management of complications of retropubic slings requires adequate knowledge of its anatomical compromise. In determining the route to be able to address the compro- mise of the mesh, the cystoscopic image is essential to define precisely the location of the extrusion. With the clinical case presented in the video, the aim is to show a management with different approaches in order to achieve the resolution of the picture.
