Enhanced recovery program in colorectal cancer: clinical outcomes and cost implementation estimation in a resource-limited hospital Programa de recuperación mejorada en cáncer colorrectal: resultados clínicos y estimación del costo de implementación en un hospital con recursos limitados
Abstract
Introduction: Colorectal cancer (CRC) is a global health issue. Standardization of its management is essential to optimize patient care. The implementation of standardized programs has shown variable economic results. Our goal is to evaluate the clinical and economic outcomes of an Enhanced Recovery Program (ERP) in CRC patients. Materials and Methods: retrospective cohort study comparing a historical group (Pre-ERP: 2014-2015) and a contemporary group that received ERP (2016-2018). Patients were older than 18 years and all underwent resective oncological surgery for CRC. Results: A total of 214 patients were included. The analysis showed a significant reduction in the median number of hospitalization days from 8 to 5 days in favor of the ERP. No increase in morbidity or readmission rates was observed. The implementation of the ERP was not associated with an increase in estimated costs. Discussion: The fin-dings are consistent with previous studies that demonstrate the benefits of ERPs in reducing hospital stay and morbidity. Importantly, this was achieved without increasing the cost of care per patient, suggesting the economic viability of ERPs in resource-limited settings. Conclusion: The implementation of the ERP maintained the quality of clinical outcomes, reduced hospital stay, and did not increase the estimated costs of care. This supports the efficiency and applicability of ERPs in similar contexts.
