Surgical Ampullectomy

Abstract

The pancreatoduodenectomy (Whipple procedure) is the stablihed surgery for the treatment of both, pancreas head and periampullary neoplasms. In the last one, the ampullectomy may be an alternative in contexts like benign lesions or lesions with malignant potencial not bigger than 3 cm. , specially in high risk surgical candidates. The ampullectomy is a procedure that consists in removing the tumor and the Vater ampolla. This incluides the resection of the back wall of the duodenum and both the distal portion of biliary duct and pancreatic duct for the posterior reimplantation. Because this is an unfrequent procedure well defined surgical steps are crucial. The video shows step by step the resolution of the case of a patient with the previous diagnostic of duodenal adenoma through a succesfull transduodenal ampullectomy.
