Abstract

INTRODUCTION: Urinary Incontinence (UI) and Overactive Bladder Syndrome (OAB) are common urological problems in the female population. These can be evaluated through the Urodynamic Study. Urodynamic diagnoses include Urinary Stress Incontinence (SUI), characterized by urine loss during valsalva maneuvers and Detrusor Hyperactivity (DH), given by the involuntary contractions of the Detrusor during the cystometric filling phase. There is also a little described entity, the Detrusor Hyperactivity secondary to Valsalva (HAD-V), which is characterized by involuntary detrusor contractions that are generated precisely after a valsalva maneuver, simulating clinically a false Urinary Stress Incontinence. The objective of this study is to determine the prevalence of Detrusor Hyperactivity secondary to Valsalva in women undergoing urodynamic study due symptoms of Urinary Incontinence and / or Overactive Bladder Syndrome. MATERIALS AND METHODS: The same operator performed a urodynamic study due to UI and / or OAB symptoms in a total of 247 patients between 2011 and 2018. Urodynamics was performed according to the recommendations of the International Continence Society. The patients were classified into groups according to their symptoms at the time of performing urodynamics: pure SUI, pure OAB, mixed urinary incontinence (IUM) with predominance of stress, predominantly urgency IUM and real IUM. An evaluation of the urodynamic records was made, associating the finding of HAD-V to each group of symptoms. RESULTS: Of a total of 247 patients, 20.2% developed HAD-V. 24 patients with pure SUI, of which 12.5% presented HAD-V 69 patients with OAB, of which 13% presented HAD-V 18 patients with IUM with predominance of SUI, of which 22.5% presented HAD-V 19 patients with IUM with predominance of urgency, of which 15.7% developed HAD-V 117 patients with real IUM, of which 26.5% developed HAD-V CONCLUSIONS: The HAD-V is a little considered and described entity. Its prevalence is not minor, and it is important to consider it, because a good diagnosis of it could help to make better therapeutic decisions in patients with symptoms of UI or OAB.
